By Bus and Train

B8: Take the B8 to 18th Avenue and Ocean Parkway.
Walk up the Service Road to end at 591 Ocean Parkway
(Between 18th and Ditmas Ave).

F Train: Take the F train to 18th Avenue. 
Walk east 5-blocks towards Ocean Parkway.
Cross Ocean Parkway and turn left onto the service road.
Walk up the Service Road to end at 591 Ocean Parkway
(Between 18th and Ditmas Ave).

Office Hours

Things to keep in mind when making an appointment

  • Routine appointments may be scheduled by calling the office during regular office hours. When you schedule your appointment, our staff will inquire about the reason for your visit, the name of your referring doctor, and your insurance information.
  • Many insurance carriers will not pay for your visit unless we have received a written referral from your primary care physician.
  • Patients are seen by appointment only.  Same day appointments and emergencies are always accommodated. Kindly notify us if you are unable to keep your scheduled appointment, so that the time might be made available to someone else.
  • Please bring all of your medications (or a list of medications with dosages) and copies you may have of your recent lab tests, imaging reports, endoscopy reports and physician notes. Please bring your drivers license or government issued ID card and your insurance cards. 
  • If you are scheduling an appointment for a procedure, please be sure to obtain and follow all written instructions.
